2. Download electronic Application Form and application checklist. Various electronic formats are available. If you have problems downloading the form through the WWW, a hard copy can be requested via e-mail (sacc2004@io.usp.br) or FAX (55.11.3091-6597, Attn.: Dr. C.A.D. Lentini). Please include a mailing address or fax number where the form should be sent.
3. Prepare all application materials described below:
(b) A brief, specific description (within space provided) of your current research, academic, or professional activities. If applicable, participation in IAI-sponsored activities should be described (please list project title, project PI, applicant's contributions).
(c) A short Statement of Interest (within space provided) explaining why you wish to participate in the Course, how you expect to benefit from participation, and how you expect to apply the knowledge gained during the Course.
(d) Request copies of official University transcripts for the highest degree you completed, showing a list of courses taken, and grades for each course. Applicants with completed graduate degrees should only make copies of transcripts from their graduate school. If you are currently in graduate school (i.e., you have not completed a graduate degree) you should prepare a complete transcript for your undergraduate degree and (b) an official University certificate showing graduate courses completed to date (with grades). Advanced undergraduate students should request an official University certificate showing graduate courses completed to date (with grades).
If you submit your application via e-mail you should keep the certificates since the organizers may request them at a later time. If you submit your application via airmail or courier enclose all transcripts and certificates in the package.
NOTE: For senior applicants, this requirement may be waived (contact Course organizers, sacc2004@io.usp.br) before the application deadline to request the waiver).
(e) A recent Curriculum Vitae (3 pages maximum), including list of publications, or a description of any research activities in which you have been directly involved. The CV should be in an ASCII text file named with your last name and extension txt (e.g. gomes.txt).
(f) If you are not a native English, Spanish or Portuguese speaker, proof of language skills may be required. This may include any Diplomas or Certificates of proficiency.
If you submit your application via e-mail you should keep the certificates since the organizers may request them at a later time. If you submit your application via airmail or courier enclose all transcripts and certificates in the package.
(g) A completed Reference Form, filled out by a person who can comment on your academic / professional abilities. The Form can be (a) mailed directly to the Course Coordinator (address is listed in the Form) or (b) included with the application materials in a sealed envelope. The Reference Form is included with the Application Form (page 5) to be retrieved electronically.
4. Go through the checklist provided with the Application Form and make sure you have all the necessary materials. Incomplete applications will not be considered!
5. Name the file containing your application form (pages 1 through 4 only) with your last name and file type (e.g. gomes.doc or gomes.rtf).
6.
Submit your application:
We
strongly encourage applicants to e-mail their application.(a) Attach your
application form (rtf file) and Curriculum Vitae (txt file) to an e-mail
and send it to sacc2004@io.usp.br
(b) If you are unable to e-mail your application send all requested materials via a courier service (e.g., DHL, Federal Express) or an express mail service. If you use a courier service, please make sure to write down (a) the airbill number (a number which uniquely identifies your package and allows you to track it), and (b) the date when you sent the package.

Applicants may be temporarily in another institution (for example, as graduate students), but they must have a permanent affiliation with their home institution and be committed to return to it within a reasonable time.
Applicants must be actively involved in scientific research. Applicants involved in activities related to the Course's subject, particularly as part of ongoing or proposed IAI-sponsored projects, have a better opportunity of being selected.
Applicants must have sufficient command of English and Spanish or Portuguese (the languages in which all course activities will be conducted) not only to understand lectures and assigned reading material, but also to participate actively in discussions and group activities.
If selected, participants will be responsible for fulfilling all legal requirements to enter Brazil.
Selected participants must commit to exclusive dedication to the Course activities during the two-week period between 22 November-3 December 2004.
